Mission Statement

Office Hours

Our patients are seen Monday through Thursday by appointment only from 8:00am to 5:00pm and we are closed for lunch from 12:00pm to 1:30pm.

Scheduling

We live in a time of demanding schedules. We are very sensitive to your needs and will do our best to offer creative scheduling for you.

Our goals are to deliver “on-time quality treatment”, to minimize your wait time, and to have plenty of time during each appointment to do the necessary adjustments and to answer any questions. As mentioned earlier, ask previous parents about the wait time in the office.

Here are some aspects you will notice:

We schedule longer appointments during school hours and shorter appointments after school. By doing this we do not reserve an entire afternoon for one or two patients, but rather accommodate as many families as possible after school. Naturally these appointments are very popular and need to be shared fairly with all our patients.

It is required by Texas’ Education Code (§ 25.087. EXCUSED ABSENCES) that school authorities regard orthodontic appointments as excused absences. Know your child’s school schedule and consider lunch and study halls for these appointments or pick a different class time each time.

Financial

Orthodontic health care is an investment in your future. The effect of a beautiful smile truly impacts you and those around you for a lifetime. Orthodontic fees vary depending upon the severity of the problem and the length of treatment and therefore are determined and discussed on an individual basis. Arrangements can be made for payment over a period of time and dental insurance plans that feature orthodontic benefits may be incorporated into payment arrangements. Our experienced Financial Coordinator will work with you to simplify the process and create a payment plan that works best for you.

Insurance

If your dental insurance covers orthodontic treatment, we will be happy to assist you in determining your plan benefits available to you and your family.

Your orthodontic insurance plan may have eligibility requirements. Some plans have age limits or waiting periods before coverage can begin.

Usually insurance policies have a lifetime orthodontic benefit that is distinct from regular dental insurance.

Total orthodontic benefits are rarely paid in one payment. Partial payments are received throughout the course of treatment until the maximum benefit has been paid. On occasion, insurance payments may be made directly to the policyholder and should be presented to our office to be applied to your account.

Please inform our office of any changes in your insurance. If the insured changes employment or the insurance coverage changes, benefits may also change or be discontinued.

We accept most PPO Insurance plans and we are an In-Network PPO Provider with: MetLife, Cigna, Delta Dental, and United Concordia’s National Fee for Service Plan

You may not see Dr. Middleton’s name as an In-Network Provider, but it if’s a PPO Plan – we accept it.

We will verify your benefits prior to the New Patient Consult appointment and notify you of the benefits.

Please remember that orthodontic insurance is a benefit for you or your child. If during the course of treatment your benefits change, the financially responsible party is accountable for all charges.

For your convenience we accept Cash, Checks, Visa, MasterCard, American Express, and Discover.
